Hospice Care
Hospice Outreach Service (HOS) is your local community hospice service.
Being diagnosed with a life limiting illness can be overwhelming. It can bring a lot of uncertainty to your life and can be difficult to navigate or to know whom to turn to for help.
How can we help you
That’s where Cancer Relief can help you. We aim to support you to live your life well by managing any symptoms you might be experiencing, as well as offering psychological and emotional support. We will work with you to achieve personal goals, gain more confidence at home or in your daily routine and help maximise your quality of life at any stage of your illness.
Our expert staff include specialist nurses, healthcare assistants, physiotherapy and holistic therapists working closely as a multi-professional team with the Gibraltar Health Authority (GHA) palliative and community care teams. HOS works alongside your GHA or private health care, and it can be accessed while you are receiving treatment such as chemotherapy or radiotherapy.
What We Offer
Cancer Relief hospice care provides vital additional person-centred care and support for anyone affected by a life-limiting cancer diagnosis throughout their illness, during end of life and into bereavement. Our specialist health care professionals’ sole purpose is to be there for you and your family; to learn what is important to you and to listen to what matters to you most relating to your care and how you want to live your life. Our services are aimed at helping you live well with your illness and supporting you to make the most of your quality of life. We do this by respecting your choices, supporting your care wishes, keeping you independent for as long as possible, maximising your comfort and upholding your dignity.
We can care for patients and their families from diagnosis, with services provided at home through one to one appointments as well as at the Cancer Relief Centre through our holistic wellbeing and day support services (anything from specialist nursing support to physiotherapy, social or financial support and counselling to complementary therapies).
What Happens Next
Many people prefer to receive support at home or remain at home during their illness. Our team of community hospice nurses and healthcare assistants, supported by therapists and GHA care teams, will keep in contact with you by phone, visiting you and your family regularly and providing reassurance and advice for physical, emotional, and practical needs including specialist hospice nursing care in the evening, during the night and over the weekends if you need it. We can also provide a wide range of essential healthcare equipment for use in your home, such as aids to assist with household tasks, recliner chairs, mobility aids, hospital beds, and aids to assist with personal hygiene needs. The equipment loan service is aimed at improving or maintaining your independence, comfort and dignity.
Services are free and available for patients, their carers or loved ones. You may be referred to one of our information or assessment nurse clinics by your GP, hospital doctor, district nurse, or any health care practitioner, or you can book in directly to see one of our nurses without a referral from a healthcare practitioner.
